Louisiana Support Coordination Software
NOW, ROW, Children's Choice, CCW & EPSDT

Support coordination agencies serving Louisiana's New Opportunities Waiver (NOW), Residential Options Waiver (ROW), Children's Choice Waiver, Supports Waiver, Community Choices Waiver (CCW), and EPSDT participants use One Care Portal to run assigned caseloads, visit and contact notes, follow-up, and supervisor review. It is the agency's working system beside the state's LaSRS, LSCIS, and EVV, not a replacement for them.

A Louisiana support coordination agency already runs its NOW, ROW, Children's Choice, CCW, and EPSDT caseloads in One Care Portal's Case Management module.

  • OIDD, OAAS and EPSDT caseloads in one place
  • Supervisor review before notes are final
  • $299/month + $7.99 per participant

Louisiana programs with support coordination

Louisiana calls waiver case management "support coordination." The Office of Intellectual and Developmental Disability Supports (OIDD, formerly OCDD) runs four waivers, the Office of Aging and Adult Services (OAAS) runs the Community Choices Waiver, and EPSDT support coordination is a Medicaid State Plan service for children and youth.

OIDD waiver

New Opportunities Waiver (NOW)

For people who meet ICF/IID level of care. Support coordination is one of the NOW services, alongside individualized and family support, day habilitation, and housing stabilization.

OIDD waiver

Residential Options Waiver (ROW)

Serves all ages with a capped individual budget set from an ICAP assessment. To stay eligible, a participant must receive support coordination and a residential waiver service every 30 days.

OIDD waiver

Children's Choice Waiver

Supplemental support for children from birth through 20 who live at home with their family or a foster family. Support coordination is a listed service; youth move to another waiver at 21.

OIDD waiver

Supports Waiver

For adults 18 and older who meet ICF/IID level of care, focused on vocational and community inclusion. LDH lists support coordination as its required case management.

OAAS waiver

Community Choices Waiver (CCW)

For older adults and people with adult-onset disabilities who meet nursing home level of care. Support coordination is a CCW service, including transition intensive support coordination.

State Plan service

EPSDT Support Coordination

For Medicaid members under 21 on the Developmental Disabilities Request for Services Registry, or ages 3 to 20 when support coordination is medically necessary ("Special Needs"). It is not a waiver service.

Read October 2026. LDH and the Louisiana Medicaid Services Manual control current requirements.

The contact rhythm a support coordinator has to keep

All three program families set minimums: at least a monthly contact and a plan reviewed during the year. These are floors, not limits; support coordinators often contact some people more often, even weekly, and document every contact. One Care Portal keeps those notes, the follow-up they create, and the supervisor's sign-off in one caseload view.

OIDD waivers (NOW, ROW, Children's Choice, Supports)

  • At least a monthly contact, by phone or face to face
  • Quarterly meeting in the participant's home, including a review of the service provider's book
  • At least one unannounced home visit a year
  • A 90-L form from the primary physician, nurse practitioner or physician assistant once a year

CCW (OAAS)

  • At least a monthly contact, by telephone, virtually, or in person
  • Quarterly plan of care review, in person or virtual
  • Annual assessment in person; annual plan of care meeting in person or virtual
  • Monthly monitoring of services such as personal assistance, with remediation when a service was not delivered

EPSDT support coordination

  • A contact at least every calendar month after linkage
  • Quarterly review of the Comprehensive Plan of Care (CPOC) with the family
  • CPOC meetings held in person at the home; some quarterly reviews may be virtual when criteria are met
  • As-needed contact until every CPOC service is in place

What stays in the state's systems

One Care Portal does not connect to, submit to, or replace any of these. Agencies keep entering what the state requires where the state requires it.

LaSRS

The state system where OAAS support coordination contacts are coded and the Support Coordination Activity Report (SCAR) is produced.

LSCIS

The Louisiana Support Coordinator Information System, where EPSDT support coordinators complete the CPOC and the CPOC Quarterly Review.

EVV

Both the OIDD and OAAS contact documentation forms require electronic visit verification for face-to-face contacts. One Care Portal does not provide EVV.

What your support coordinators get in One Care Portal

This is the existing Case Management module, set up per agency and per program. It is not a separate Louisiana product.

One caseload across programs

Role-scoped caseloads, calendars and visit schedules, with NOW, ROW, Children's Choice, Supports Waiver, CCW and EPSDT participants side by side.

A note for every contact

Weekly, monthly or quarterly, each note records date, time, method, type, location and people present, with goal and service monitoring, health and safety review, and incident and hospitalization review inside the note.

Follow-up that carries forward

Problems found at a contact stay open on the next note until someone resolves them.

Supervisor review

Required-field validation, signatures, attestation, amendments and audit history, with supervisor approval or return with feedback.

Authorization records

Service authorization records and an expiring-authorizations report.

Documents with expiration dates

Participant and staff documents, such as the annual 90-L, stored with expiration dates and reported before they lapse.

Boundaries

  • No connection to LaSRS, LSCIS, the Louisiana Medicaid fiscal intermediary, or any state system.
  • Not certified, endorsed, or approved by LDH, OIDD, or OAAS.
  • No electronic visit verification, GPS capture, or installed mobile app. It runs in the browser.
  • No pre-built official Louisiana forms. Note sections and required fields are set up with your agency.
  • No automatic enforcement of contact cadence. Your team still schedules and tracks it.

Frequently asked questions

Do Louisiana support coordination agencies use One Care Portal?

Yes. A Louisiana support coordination agency runs its NOW, ROW, Children's Choice, CCW, and EPSDT caseloads in One Care Portal's Case Management module. Supports Waiver caseloads fit the same workflow.

Does One Care Portal connect to LaSRS or LSCIS?

No. It is the agency's working system for caseloads, notes, follow-up and supervisor review. Anything the state requires in LaSRS or LSCIS is still entered there.

Does it replace EVV for face-to-face visits?

No. Louisiana's contact documentation forms require EVV for face-to-face support coordination contacts. One Care Portal does not provide EVV.

Can one agency manage OIDD waiver, CCW, and EPSDT participants together?

Yes. Participants from each program sit in the same caseload, and note sections and required fields are set up per program during onboarding.

What does it cost?

$299 a month for the platform plus $7.99 per active participant, month to month. Onboarding scope is reviewed during the demo.
