When a participant’s needs change, the next coordinator should be able to see the current plan, what happened at the last contact, and what still needs follow-up. One Care Portal gives Michigan supports coordination teams a shared place to manage those records.
MI Choice waiver agencies assign supports coordinators to help develop the person-centered service plan. Coordinators monitor the plan and help arrange the supports chosen by the participant. Approved MI Choice waiver, Appendix D.
Assessment submissions to Michigan
MDHHS directs MI Choice waiver agencies to submit assessment information to the state in files. Find the Home Care Submission Manuals under Submission Manuals on the provider resource page, alongside the MI Choice chapter of the Medicaid Provider Manual. MDHHS MI Choice provider resources.
Internal workflow example. If a participant reports that a service is no longer meeting their needs, connect the contact note to the affected plan item, record the proposed follow-up, and assign someone to verify the outcome.

Keep follow-ups visible across your caseload
Use the modules together or start with the workflow your agency needs most.
Case Management
Manage caseloads, participant plans, contact notes, and service authorizations. Carry open follow-ups into later notes and give supervisors a place to approve notes or return them with feedback.
Give supervisors a consistent way to check readable PDF notes and plans against agency criteria. Reviewers can inspect flagged gaps and keep a record of their decisions.
Track what your billing team submitted, which claims need attention, and what has been paid. Validate claims, work denials, and post 835 electronic remittances where applicable.
Can we use Compliance Review alongside our coordination system?
Yes. Keep your EHR and upload readable PDFs for review against agency criteria. Automated imports depend on compatible EHR API access and are configured during onboarding.
How can we check which Michigan claims our team can submit?
Claim submission uses supported clearinghouse workflows. We confirm payer, plan, enrollment, transaction, and claim-type fit during setup, based on the services and payers your agency handles.
