Texas behavioral-health documentation QA

Review Texas mental health TCM documentation before billing

Keep your EHR and add a repeatable supervisor review alongside it. Start with the requirements that apply to your mental health targeted case management (MHTCM) program, check the records your team has, and let a qualified reviewer decide what needs correction before the separate billing step.

Start with the right program

“TCM” can refer to different benefits and populations. This guide focuses on Texas Medicaid mental health targeted case management, not every Texas case-management service. Before using a checklist, confirm the service, date, payer contract and current rule version with your compliance or billing lead. The Texas Medicaid Provider Procedures Manual (TMPPM), Behavioral Health and Case Management Services Handbook, has a dedicated MHTCM Documentation Requirements section (§5.2.2.6), checked October 28, 2026; TMHP updates the manual regularly, so confirm the current edition. For Texas Administrative Code text in effect, use the Secretary of State's current TAC portal; the manual, TAC, payer directions and agency policy are separate sources to reconcile, not interchangeable checklists.

Questions to bring to a review

The TMPPM MHTCM section describes diagnosis and service-need documentation, records of services and attempts, and details of the case-management encounter. A reviewer might ask:

  1. Is the diagnosis and need for MHTCM documented in the record?
  2. For routine or intensive case management as applicable, do the assessment, plan, goals and service needs connect to what the case manager did?
  3. Does the encounter record identify the reason for contact, people or entities involved, intervention, progress toward relevant goals and needed-service timeline?
  4. Are the service date, start and end time, delivery mode, agency, employee signature and credentials recorded where the applicable guidance requires them?
  5. If service was attempted, declined or delivered by telehealth, has the team checked the additional documentation that applies?

These review prompts based on the manual are a starting point. Inspect the authoritative source record and current payer instructions so qualified staff can make the final decision in context.

A human QA workflow before the billing handoff

  1. Choose the applicable criteria. Have the agency's program lead map current Texas authority, payer requirements and internal QA policy to the service and document type.
  2. Check the record. Review available visit notes, assessments and chart material for possible gaps, with the underlying document and page evidence at hand.
  3. Resolve with the right person. A supervisor or qualified reviewer confirms or dismisses the concern, asks the staff member to correct the source record if needed, and records the disposition.
  4. Hand off separately. Billing staff make their own decision under the applicable rules. The billing team applies current payer rules to the complete record.

Keep your EHR. Add a review layer.

One Care Portal Compliance Review can review eligible readable PDF visit notes, chart records, assessments and other documents against criteria your agency configures by program. Each PDF is analyzed separately; reviewers inspect cited-page findings and record human decisions. Start with PDF upload on day one; if your EHR offers API access, we build the connection during onboarding so documents flow into review automatically. Your team sets and verifies the criteria; qualified reviewers make the final decision, and your EHR remains the source record.

Official sources and scope

Use these educational prompts in your agency's review process. Qualified program and billing staff validate each criterion against current authority and the source record, then make the final compliance and claim decisions.

Frequently asked questions

How are Texas TCM review criteria set up?

Your program lead configures the criteria for your agency's MHTCM services, payer and QA process. Qualified reviewers confirm them against current requirements.

Yes. Your EHR stays your system of record. Start reviewing PDF exports on day one; if your EHR offers API access, we build the connection during onboarding so documents flow into review automatically.

Who decides when a TCM claim is ready for billing?

Your qualified reviewer checks possible documentation gaps; your billing team makes the separate claim decision under current payer rules.
