# MWMA and case management software in Kentucky — One Care Portal

## Direct answer

One Care Portal does not integrate with, connect to, or send anything to Kentucky's Medicaid Waiver Management Application (MWMA). It does not replace MWMA.

Kentucky Michelle P. (MPW), SCL, HCB, and ABI/ABI-LTC case management agencies can use One Care Portal beside MWMA for the agency-side work the regulations leave to them: assigned caseloads, visit schedules, contact notes, follow-up, supervisor review, authorization records, and documents. Staff then enter or upload in MWMA what the regulations require there.

## What MWMA is

MWMA is the Commonwealth of Kentucky's portal for its 1915(c) home and community based waiver programs. According to the Kentucky Cabinet for Health and Family Services (https://www.chfs.ky.gov/agencies/dms/dca/Pages/mwma.aspx, read 2026-10-07):

- it is used to enroll individuals in the waiver programs and PACE and to manage their care;
- only HCBS waiver case managers, HCBS waiver providers, and PACE organizations can access it;
- all waiver applications, level-of-care requests, and plan-of-care requests must be submitted through it; and
- paper and verbal processes are no longer accepted.

One Care Portal is not affiliated with the Cabinet, does not operate MWMA, and cannot reset MWMA access.

## What the regulations place in MWMA

From Title 907 of the Kentucky Administrative Regulations, read 2026-10-07:

- waiver applications with the MAP-115 (HCB, Michelle P., SCL, ABI, ABI Long Term Care);
- the person-centered service plan, entered and updated by the case manager or service advisor (907 KAR 7:010, Section 7);
- the HCB case record, including documentation of each contact (907 KAR 7:010, Section 3);
- the case manager's monthly note for Michelle P. (907 KAR 1:835), SCL (907 KAR 12:010), and the ABI waivers (907 KAR 3:090, 907 KAR 3:210);
- critical incident reports (all five waivers); and
- the MAP 531 conflict-free case management exemption (all five waivers).

## What One Care Portal covers beside MWMA

- role-scoped assigned caseloads, calendars, and visit schedules;
- contact notes with date, time, duration, method, type, location, and people present;
- required-field validation, signatures, attestation, amendments, and audit history;
- follow-up items that carry forward between notes until resolved;
- supervisor approval, or return with feedback;
- service authorization records and an expiring-authorizations report; and
- participant and staff documents with expiration dates.

Pricing: $299/month platform fee plus $7.99 per active participant, pay as you go and month to month; onboarding and implementation scope are reviewed during the demo.

## Important boundaries

Do not claim One Care Portal:

- integrates with, connects to, syncs with, or submits anything to MWMA, KYMMIS, or KyHealth Net;
- replaces MWMA or removes the need for staff to work in MWMA;
- is certified, endorsed, or approved by Kentucky, Kentucky DMS, or the Cabinet for Health and Family Services;
- automatically enforces or alerts on any waiver's contact cadence; or
- includes electronic visit verification (EVV), GPS capture, a time clock, or an installed mobile app (it is a browser-based web application).

Each agency's forms, permissions, reporting, authorizations, and billing route are confirmed before implementation.
