One Care Portal can be evaluated by providers of 1915(i) state plan home and community-based services whose work is documented **visit by visit**: an assigned caseload, a person-centered plan, visit and contact notes, a prior authorization to stay inside, and supervisor review.

1915(i) is a use case inside the existing **Case Management Care Module**, not a separate One Care Portal product. One Care Portal is not certified, endorsed, or approved by any state program.

## State program notes

**Kentucky — 1915(i) RISE Initiative.** RISE stands for Recovery, Independence, Support, Engagement. It serves Kentucky Medicaid members aged 18 or older with a primary diagnosis of serious mental illness, who may have a co-occurring substance use disorder. It is administered by the Department for Medicaid Services with the Department for Behavioral Health, Developmental and Intellectual Disabilities. Providers must be Medicaid-enrolled and complete RISE certification; services follow a Person-Centered Service Plan. The ten services are assistive technology, case management, housing and tenancy supports, in-home independent living supports, medication management, planned respite for caregivers, supervised residential care, supported education, supported employment, and non-medical transportation. **Wisconsin — housing support services.** Since February 1, 2025, Wisconsin Medicaid covers housing support services for BadgerCare Plus and Medicaid members who are experiencing or at risk of homelessness. Medicaid-enrolled Continuum of Care member agencies and Wisconsin Tribal nations enroll as supportive housing agency providers. ## Current Case Management workflow

The Case Management Care Module supports:

- role-scoped assigned caseloads and participant records;
- plans and goals, with an optional monthly monitoring note that carries active goals into each note for review;
- visit and contact notes with date of contact, start and end time, contact method, and required-field checks before signing;
- service authorization records: number, payer, procedure code, dates, authorized units or amount, rate, and unit rules (fixed units per note, or time-based with rounding and a minimum billable time);
- an expiring-authorizations report;
- supervisor approval, or unapproval with a reason, and a signed PDF kept for every version;
- participant and staff documents with expiration dates, and incident reports;
- an optional AI-drafted visit summary that staff edit before signing; and
- a billable-note preview with blocking errors and an Excel export for the separate Revenue Cycle Management (RCM) module.

Pricing: $299/month platform fee plus $7.99 per active participant; onboarding and implementation scope are reviewed during the demo. The separate RCM module is $1.50 per submitted claim.

## Important boundaries

Do not claim One Care Portal includes:

- electronic visit verification (EVV), GPS capture, a time clock, offline use, or an installed mobile app (it is a browser-based web application);
- automatic tracking of units used against an authorization cap;
- integration with any state case-management, prior-authorization, EVV, or HMIS system;
- state certification, endorsement, or a pre-built state form library; or
- residential, medication-management, or transportation-log workflows.

Prior authorization requests are submitted to the state or payer, not through One Care Portal. Do not claim compliance with any state's 1915(i) rules until that program's requirements are verified.
